The West End location of the Physical Medicine Center is located on the corner of West Broad Street and North Crestwood Avenue. Heading west on West Broad Street, pass the intersection of Libbie Avenue and West Broad Street. North Crestwood Avenue is the fourth street to the right after Libbie Avenue. Turn right on North Crestwood Avenue and either park on the right or pull in behind the building for more parking.

Physical Medicine and Rehabilitation: 8:30 am to 6:00 pm Tuesday and Thursday, and 1:00 pm to 6:00 pm Friday

Charles H. Bonner, M.D., M.S., C.R.P., Medical Director: 8:30 am to 5 pm Tuesday and Thursday, and 1:00 pm to 5:00 pm Friday

Ann Spaulding, P.A.-C.: 8:30 am to 5:00 pm Monday

Closed from noon to 1:00 pm for lunch

The Midlothian location of the Physical Medicine Center is located West of the Midlothian Turnpike and Arch Road intersection. It can also be reached from Arch Road by turning south on Arch Road (stay in the left lane) and making the first right on Knightsbridge Road (Archway Sixty). Make the first right into the complex and keep right. The side of building 9323 will be on your left as you go through the parking lot (keep right). As you come around to the front of the building, Suite R is on your left. Park and enter there.
